To keep the ladder interesting you need automatically drop people ranks after a week or two. For top five if you are inactive a week you should drop 10 ranks.

i'm unable to play for a while. Broke my wrist to the point of needing hardware put in. Right now i can barely move it, much less play an rts.

But that got me thinking. What if someone had hardware put in their hand/wrist and it ended up doubling their APM. Would that be a form of hacking?

Bionic hands are NOT bannable as long as it's the hand itself operating a standard mouse at an accelerated speed, and not some direct hand-to-computer bluetooth dongle communication type thing.
